I offer the opportunity for us to see if it would be helpful for us to work together...simply Call Me for a Free 15 minute phone consultation!
I offer individual, family, and couples therapy. Please contact me and I would be happy to discuss insurance coverage further. It is your responsibility to check with your insurance company to ensure coverage of services with your specific plan. It is also your responsibility to obtain any pre-authorizations for service that may be required by your insurance company. I do not bill insurance for out of network coverage. However, I will provide you with a billing statement upon request which you may submit to your insurance company. I cannot guarantee reimbursement.
Currently these are the Insurance companies I am a provider. Again it is important to contact your insurance company regarding coverage as coverage may vary by plan.
No Surprises Act
The No Surprises Act is aimed at eliminating surprise medical bills for patients. In regards to your work with me as a mental health provider, if you are not using your insurance to cover services you have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
- You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs for therapy sessions or any other services related to your treatment with me.
- Make sure your health care provider gives you a Good Faith Estimate in writing at least 1 business day before your appointment. You can also ask your health care provider, and any other provider you choose, for a Good Faith Estimate before you schedule an item or service.
- If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill.
- Make sure to save a copy or picture of your Good Faith Estimate.
For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises or contact Krystal Davis at 425-298-3844 or krystal@krystaldavisllc.com
